This is an ADHD Assessment Checklist that many doctors have their patients fill out. I remember filling out one like this. I thought it was going to take a week to fill out. You can print this page out and take this to the doctor with you.
Your doctor may have you fill out his/her own ADHD Assessment Checklist while you are in the office.
Remember, this is for things that are beyond what would be normal for the age of the patient.
